Eligible Service Periods

·        World War II                                             12/7/1941 through 12/31/1946

·        Korea                                                        6/27/1950 through 1/31/1955

·        Vietnam                                                      8/5/1964 through 5/7/1975

·        Grenada                                                     8/24/1982 through 7/31/1984

·        Panama                                                      12/20/1989 through 1/31/1990

 

*Must have been honorably discharged.

*Veterans complete VA Form 21-526.

*Widows of deceased veterans complete VA Form 21-534.

*Medical expenses are reported on VA Form 21-8416.

*Doctors must complete “Need for Regular Aid & Attendance”, VA Form 21-2680

 

You will need the following

 

*DD 214 MILITARY DISCHARGE PAPER – If you cannot locate the original, obtain a new copy by going on the VA web site at www.vetrecs.archives.gov .

*VETERANS SS# & DATE OF BIRTH- If applying for a widow of a veteran; her social security number, date of birth, address, next of kin or POA information.

*MEDICAL EXPENSES- i.e. secondary insurance, pharmacy, medical supplies, personal care home rent, private aides, or any non reimbursable medical expenses. (see attached)

*NAMES & ADDRESSES OF PCP & FACILITY they are residing in.

*INCOME INFORMATION- Social Security, pensions, investments, interest, additional income, etc.

*FOR WIDOW APPLICANTS- A copy of the MARRIAGE CERTIFICATE and the DEATH CERTIFICATE OF THE VETERAN.
