Our Process for Proper Senior Care Placement
Initial Intake
Every family’s situation is unique. During our first conversation, we’ll take the time to get to know your loved one, understand your concerns, answer your questions, and learn what is most important to your family. Whether you’re planning ahead of facing an unexpected change, we’re here to provide guidance, support and a clear path forward.
We listen, we learn and we ask pertinent questions to implement a “game plan.”
Personal Evaluation
Every family's situation is different, so we tailor our evaluation to your individual needs. We gather information about your loved one's care needs, cognitive and physical abilities, daily routines, geographic preferences, and what is most important to your loved one and you.
Depending on the situation, this may include meeting with your loved one or family in person, or gathering the information we need through our phone conversation if the situation is more urgent. When an in-person visit is helpful, we can meet in the home, hospital, or skilled facililty.
This process is all about making it convenient for you.
Research & Recommendations
Over the years we’ve built strong relationships with local senior living communities and have done the research for you. We know each community’s strengths, care capabilities, pricing and availability.
Once we have gathered all the necessary information, we consider medical, financial, and geographical criteria to provide your family with several “best fit” options to consider. There are plenty of senior living options out there, and our research saves you valuable time and frustration. We will schedule and accompany you to tour each community and explore any financial assistance to offset the cost of care (see Veteran's Benefits info below).
We bridge trust and lend support to ensure you make a confident, well-informed decision.
Facilitate the Move
Our job does not end once you've made your senior placement decision. We will maintain communication with the community chosen to provide them with what they need. We complete and deliver all necessary paperwork from our assessment, fax medical forms, and keep the lines of communication open with all parties before, during and after the move.
We remain by your side to support you through the transition.
